Forms & Docs
BSA Medical Forms (A&B)
Scouts BSA requires that you submit an up-to-date and valid record of your medical history. Every person attending camp (adults, staff, and youth) must have at least PART A & B of these medical forms on file with the camp staff or they won’t be allowed to attend Camp Tomahawk.
Prescription Drug Authorization Form
Our medically trained adult staff can handle administration of prescription drugs to your Scout. HOWEVER, every Scout requiring at-camp prescription medication MUST have the State of Maryland Prescription Drug Authorization Form completed and submitted to the camp staff or they won’t be allowed to attend Camp Tomahawk.
Non-Prescription Authorization Form
The excitement and fun of summer camp has the potential to result in a bump or scrape along the way. We need permission to treat your son or daughter if that happens. Every youth attending Camp Tomahawk MUST have this form completed and submitted to the camp staff or they won’t be allowed to attend Camp Tomahawk.
